Sotalolem indukovaná setrvalá komorová tachykardie u nemocné s pokročilým selháním ledvin

Autoři předkládají kazuistiku 65leté nemocné s pokročilým selháním ledvin na podkladě diabetické nefropatie, u níž byla v roce 1998 zahájena terapie redukovanou dávkou sotalolu z indikace paroxyzmální supraventrikulární tachykardie a fibrilace síní. U pacientky došlo navzdory použití redukované dávky sotalolu k prodloužení QTc intervalu a k výskytu setrvalé monomorfní a polymorfní komorové tachykardie, která vyžadovala opakovanou farmakologickou a elektrickou kardioverzi. Po přerušení terapie sotalolem a opakovaných hemodialýzách došlo u nemocné ke zkrácení QTc intervalu a vymizení epizod komorové tachykardie.
