Botulotoxin v léčbě vertebrogenních bolestí

U šesti pacientů s chronifikovanými a recidivujícími vertebrogenními potížemi typu cervikobrachiálního (CB) syndromu a lumbaga byla provedena aplikace botulotoxinu (Botox-) do svalů se zvýšeným svalovým napětím a dle klinického vyšetření zapojených do vyvolávání bolestivého syndromu. Aplikováno bylo 30-80 MU rozdělených do 2-4 míst. U 5 ze 6 pacientů došlo k výrazné úlevě potíží na dobu několika týdnů až měsíců. U jednoho pacienta po aplikaci došlo po přechodné několikadenní úlevě ke změně potíží (lumbago přešlo v lumbosakrální (LS) syndrom s iritací L5). Získaná klinická zkušenost svědčí pro přínos botulotoxinu pro léčbu chronifikovaných vertebrogenních potíží, pokud lze léčebný přínos očekávat od dlouhodobé cílené myorelaxace svalů podílejících se svou hyperaktivitou na vyvolávání bolestivého syndromu. Pro skutečné zobecnění závěrů z přínosu léčby botulotoxinem při řešení chronických a recidivujících vertebrogenních bolestivých syndromů je ovšem zapotřebí rozsáhlejší studie a klinická zkušenost.
