Intravaskulární ultrazvuk (IVUS) je vyšetřovací metoda na rozdíl od angiografie zobrazující lumen i stěnu koronární tepny. Tato výhoda umožňuje diagnostikovat proces koronární aterosklerózy již v časných stadiích, kdy ještě nedochází ke změnám kontury tepenného lumen a kdy je proto nelze detekovat angiograficky. Z hlediska současné diagnostické praxe je IVUS užíván při indikacích ke koronárním intervencím, kdy u nejasných nálezů je vhodným doplňkem angiografie. Ačkoliv rutinní periprocedurální IVUS vyšetření není nezbytné, jeho aplikace napomáhá optimalizovat výsledky perkutánních koronárních intervencí (PCI), zejména v případě implantace stentů. V diagnostice transplantační koronární vaskulopatie je IVUS vyšetřením volby díky své schopnosti diagnostikovat onemocnění podstatně dříve než angiografie.

Intravascular ultrasound - aplication in cardiology

Unlike angiography, intravascular ultrasound (IVUS) is an examination method enabling to visualise coronary artery lumen as well as the wall. This advantage allows to diagnose the process of coronary atherosclerosis in early stages when the changes of the arterial lumen contour are not yet obvious, which prevents their detection by angiography. From view of the present diagnostic practice, IVUS is used in the cases of indications to coronary interventions, when in disputable findings proves itself as a useful supplement to angiography. Although routine periprocedural IVUS examination is not necessary, its application helps to optimize the results of percutaneous coronary interventions (PCI), especially in the cases of stent implantations. Due to its ability to identify the process significantly earlier than angiography, IVUS is a method of choice in diagnostics of the coronary transplant vasculopathy.
